• Application and Fee,
• Official transcript from advanced practice program, with degree and date posted,
• National Certification,
• State background check, and
• Federal background check

• Must be from your advanced practice program and include the degree and the date the degree was posted.

• Must provide KBN with proof of national certification.

• Request a background check from the Administrative Office of the Courts (AOC).
• The report must come form AOC.  No other state reports will be accepted. 
• Instructions for obtaining the report [PDF Format - 187KB] are available to assist you.  Print and follow the instructions.

• Request a fingerprint card.
• Send the completed fingerprint card and the $16.50 processing fee to the Kentucky State Police.

REPORTING A FELONY OR MISDEMEANOR CONVICTION:

• Review information regarding the convictions that must be reported and the required documents to be submitted to KBN.
• Fax letters of explanation (see above link) to 502-429-3353.
• Certified court documents must be mailed to KBN, Investigation and Discipline Branch
  • It could take up to 3 months to process applications with a "Yes" response to any discipline and/or conviction question.

NOTE:  If you answer "yes" in error to a disciplinary and/or criminal history question you may fax correction documents to 502-429-3336.

ANSWERED THE CONVICTION OR DISCIPLINARY QUESTION IN ERROR?

Send a letter of correction to:

• Fax to 502-429-3336.
REQUIRED CONTINUING EDUCATION
• Two hours of HIV/AIDS continuing education [PDF Format] required before licensure, and
• Three hours of Domestic Violence continuing education [PDF Format] within 3 years of KY licensure
• One and a half hours of Pediatric Head Abuse Trauma [PDF Format] continuing education within 3 years of KY licensure.
