The online license validation system is the most efficient and economical method of verification. However, KBN does offer other methods, as listed below.

Validation of licensure in a list format is available at a cost of $10 for the first record and $1 per record thereafter. Include either the social security number or the nursing license number of the individual(s) to be verified and mail the list along with the appropriate fee to:

Verification of Original Licensure to Other State Boards |

This method of verification is to be completed by nurses who have or have held a Kentucky nursing license and/or advanced practice registration and are seeking licensure in another state or jurisdiction.
Complete the Verification of Licensure to Other Boards of Nursing Form, [PDF Format - 19k] and mail the form, along with the required $50 fee (check or money order made payable to the Kentucky Board of Nursing), to:

Upon receipt of this request and fee, a verification of the nurse's Kentucky license and ARNP registration, if relevant, will be sent to the state board indicated on the request form (please allow a minimum of 4 weeks processing time).
