Licensed Nurse Address or Email Change
Nursing licensees can routinely update their mailing address or email address online by clicking on the link below, and following the instructions on the menu screen:
Note: When changing your address online, please do NOT use any special characters in the address change, such as commas, periods, hyphens, slash marks, or number signs, etc. Online address changes are to be inserted on the first line of the address section; only use line 2 of the address change when needed.
Please note that, at this time, Dialysis Technicians cannot change their address online. Click on the link to the right for additional information.
Kentucky Revised Statute 314.107 and Kentucky Administrative Regulation 201 KAR 20:370, Section 1(11), require a licensee to notify KBN upon establishment of a new mailing address. Effective immediately, for security purposes, KBN will no longer accept address changes via electronic mail. In order for your nursing license record to be updated, you must either submit the change online, or complete an Address Change Form [PDF Format - 14k] and mail it to the address indicated on the form.

Alternate methods of changing your address are listed below:
- Via written mail by completing the Address Change Form [PDF Format - 14k] and returning it to the KBN address provided on the form.
- Via written mail by submitting your address change, along with your nursing license number or social security number to Sharon Minniefield, Licensure Specialist, Kentucky Board of Nursing, 312 Whittington Pky, Ste 300, Louisville, KY 40222-5172.
