Verification of Original Kentucky Licensure

If you are a RN or LPN and want to verify your original Kentucky nurse’s license data to another state where you are applying for a license, you must go to the Nursys™  website. 

Nursys™ (Nurse System) is a secure computer system that contains nurse licensure and licensure disciplinary information that is provided by boards of nursing in the United States and its territories.  

To verify your original Kentucky licensure data to any entity other than another state board of nursing,  you must send a written request form [PDF Format - 18k] plus the $50 fee to KBN.  Allow 4 weeks processing time. 

To validate that an individual holds a current, active nursing license, ARNP registration, or SANE credential:

• Use the KBN Online Validation System 
  
  
• Send a written request with either the social security number or license number of the individual(s) whose license(s) you wish validated and the appropriate fee to KBN.  The cost of validating current licensure is $50 for the first record and $20 for each additional record.
