Testing Modifications for NCLEX Candidates with Disabilities

Special Accommodations must be requested from the Kentucky Board of Nursing.  It is recommended that the request and documentation supporting the disability be submitted  with the application for licensure.  Supporting documentation must include the following:

  1. Letter from candidate requesting accommodations and a completed NCLEX Special Accommodations Request Form [PDF Format - 25k].
  2. Letter from the nursing program administrator defining what accommodations the candidate was afforded during the educational process.
  3. A professionally recognized diagnosis must be included in the documentation which the board receives from the candidate.  Documentation must also include:
    • A history of the disability and any past accommodation granted the candidate and a description of its impact on the individual's functioning;
    • Identification of the specific standardized and professionally recognized test/assessments give (e.g., Woodcock-Johnson, Weschler Adult Intelligence Scale,);
    • The scores resulting from testing, interpretation of the scores and evaluations;
    • Recommendations for testing accommodations with a stated rationale as to why the requested accommodation is necessary and appropriate for the diagnosed disability.
  4. Supportive documentation must include recent reports, test results, evaluations and assessments of the candidate's need for accommodations due to a disability (physical or mental impairment) that substantially limits one or more major life activities.

KBN will contact the candidate if the request has been denied.
