Criminal Background Report (CourtNet)

You must submit a criminal background report with your application for licensure.

To obtain a criminal history report, complete the Criminal Background Request Form (CourtNet) [PDF Format - 13k], and submit it as directed on the form. 

• If you apply online, the Administrative Office of the Courts will send copies of your criminal history report to KBN electronically.  You do not have to fax or mail copies.
  
  
• If you apply using a paper application, you must submit copies of your criminal history report WITH YOUR APPLICATION.  The criminal history request form is included when your download the Examination Appliction Form.

Your application will not be processed, and you will not be issued a Provisional License until the criminal history report(s) is received by KBN.

A criminal history report is needed for every name you have used, including your maiden name and any/all married names.
