Validation of Original Licensure from Another State

Applicants for licensure by endorsement are required to submit verification of licensure information from the nursing board that issued your initial nursing license based on taking the State Board Test Pool Examination (SBTPE) or the National Council Licensure Examination (NCLEX).

The acceptable methods of requesting verification are listed below.

Verification Requirements

There are two methods of obtaining this verification (a fee is required with either method):

  1. Through Nursys External Link - You are now leaving the .gov domain.

    Nursys™ (Nurse System) is a computer system that contains nurse licensure and licensure disciplinary information that is provided by boards of nursing in the United States and its territories (Kentucky is a participating state).  Click on Nursys Licensure Verification External Link - You are now leaving the .gov domain. and follow the instructions provided there.  You can use a credit card to request the verification online or a paper form is available (fee information and instructions for both methods are found on the above link).

  2. Directly from the nursing board that issued the original license.

    If your state of original licensure is not a Nursys™ participant, you must complete the form entitled Verification of Licensure (Attachment 2) [PDF Format - 25k].  If you have already downloaded the licensure application form with the instruction sheet, this form was included in the download.  Complete the form and mail it to the nursing board that issued your original nursing license.  Be sure to contact that board for the appropriate fee amount.  For a complete list of nursing boards in the United States and its territories, go to Nursing Board ContactsExternal Link - You are now leaving the .gov domain.
