Licensure By Endorsement

If you have a license in another state or U.S. jurisdiction, and you want to obtain a Kentucky nursing license, you need to apply for licensure by endorsement.  Downloadable forms and instructions are available below.

You can check the status of your application from the following secure site:

RN or LPN endorsement applicants must submit the following:

• You can now complete the Endorsement Application online by clicking on UNIVERSAL APPLICATIONS and following the instructions provided.
  
  
• $150 application fee.
  
  
• Fingerprint Card with $19.25 fee.
  
  
• Criminal History Reporting (section 5 of the application involves reporting of any criminal history).    ...more info
  
  
• Proof of meeting the Endorsement Competency (CE) Requirement, including HIV/AIDS education.
  
  
• Validation of Original Licensure from another entity.
  
  
• Criminal History Report from the FBI (allow 4-8 weeks for
  processing).
  
  
• Transcripts are needed if you graduated from any of the following states program of nursing:
  
  California 
  Illinois
  Connecticut
  Kansas
  Washington

Note:  A transcript must be recieved from any school in the state of California before a temporary work permit is issued.
