The KBN Credentials Branch is responsible for licensing practical nurses and registered nurses, and for credentialing advanced registered nurse practitioners and sexual assault nurse examiners.

You can now submit UNIVERSAL APPLICATIONS online by clicking on the link and following the instructions provided. Please note the methods of licensure below (click on the links for additional information), and select the appropriate one when you access the online application system.
NOTE: If you are an Advanced Registered Nurse Practitioner (ARNP), you must submit a paper application for your RN Licensure and ARNP Registration. Go to the ARNP Credential page to print the paper application, instructions, and additional forms required.

The paper application for licensure is for both the licensed practical nurse and the registered nurse. Complete all sections as outlined in the instructions, typing or printing in black ink. An incomplete application will be returned to you.
Select the appropriate method of licensure from the list below, then click on the link to print the paper application, instructions, and additional forms required.
Licensure fees and regulatory requirements are subject to change. Application fees are NON-REFUNDABLE. For a current list of fees, refer to Fees for Licensure and Registration Applications and Services [PDF Format - 26k].

Validation of Licensure/Registration |
