A name and/or address change may be made online or by mail. The two procedures for this are provided below.

To change your name and/or address online, your name, social security number and date of birth must be currently listed on the registry. If any of this information is not on the registry, you will not be able to make the changes online.
Note: When changing your address online, please DO NOT use any special characters in the address change, such as commas, periods, hyphens, slash marks, or number signs, etc. Online address changes are to be inserted on the first line of the address section; only use line 2 of the address change when needed.
