Data Roster Download

To obtain addresses for nurses or state registered nurse aides, complete the Data Roster Download Request Form and pay the $500 fee. The fee must be paid via credit card or ACH Transaction.

Orders will be sent via email.

Requests are typically filled within 3-6 business days.

How Do I sign up?

There is no sign up. All users may access the service by completing the Data roster Download Request Form.

How Do I use the service?

Use the link above to submit an email to KBN after making selections and providing payment.

Within 4-6 weeks your data will be emailed to you as an attachment to the email used during payment unless otherwise specified in the comments.

What Format is the Data Provided In?

Data is provided in Excel (.xls) format or semicolon-delimited format (.csv) upon request.

What Data is Included in the Download?

KBN does not provide any additional data that is not listed below at this time.

Important Notes

Social security numbers, Date of Birth, telephone numbers, and email addresses are NOT included in the download.
Licensees are responsible for updating their own information. Therefore, the accuracy of data on file cannot be guaranteed by KBN.
